England all-rounder Andrew Flintoff missed the sixth one-day match against India on Wednesday because of the latest injury to his left ankle.
The 29-year-old, who has had three operations on the ankle, saw his surgeon on Tuesday evening.
His place in Saturday's final match and the World Twenty20 is in doubt and England will make an announcement on his future later on Wednesday.
Fellow all-rounder Ravi Bopara has already been ruled out of the Twenty20.
Bopara suffered a broken thumb in the fifth match of the one-day series, which India won to make the score 3-2 in England's favour.
Seamer Chris Tremlett is also a doubt for the Twenty20 tournament, which starts on 11 September, after having an injection in an inflamed ligament in his right ankle and will be reassessed on Thursday.
England will wait for fitness updates on Flintoff and Tremlett before naming any replacements.
The 15-man squad fly out to South Africa on Sunday following the final match of the series against India at Lord's on Saturday.
